What bacteria causes fishy odor?
Trimethylaminuria, also known as fish odor syndrome, is a metabolic disease caused by a defect in the liver enzyme flavinmonooxygenase 3 (FMO3). This enzyme is responsible for metabolizing trimethylamine (TMA), which is primarily produced by the degradation of choline and trimethylamino N-oxide (TMANO) by intestinal bacteria. Excess TMA is eliminated through body secretions, such as urine, sweat, and exhaled air, causing a characteristic fishy odor.
There are two forms of trimethylaminuria: primary and secondary. The primary form is caused by a genetic defect in the FMO3 gene, which is inherited in an autosomal recessive manner. The secondary form occurs due to damage to liver or kidney function.
The first symptoms of bad odor usually appear in childhood, especially when consuming foods rich in choline and TMANO. Although trimethylaminuria does not affect psychomotor development or growth, bad odor can have a great impact on social relationships, causing low self-esteem, anxiety, depression and even suicide attempts due to the rejection they may experience in their environment.
The diagnosis of trimethylaminuria is made in two steps. First, excess TMA is determined using biochemical tests in the urine, such as determining the TMANO/TMA ratio. This ratio will be decreased in patients with trimethylaminuria. Other alternative methods have also been found to demonstrate TMA excess, such as breath study and urine analysis using stable isotope dilution. Once excess TMA is confirmed, a molecular genotype study of the FMO3 gene is performed to detect the mutation associated with this disease.
Although there is no cure for trimethylaminuria, there are therapeutic options that can dramatically improve patients’ quality of life. The approach to this disorder is based on reducing and hiding the bad odor due to the social repercussions it causes. Regular body hygiene and washing clothes with acidified soap are recommended. In cases of intense odor or periods of stress that increase sweating, drugs can be used to reduce the concentration of intestinal bacteria, such as antibiotics, laxatives or ion exchange resins. Regarding diet, it is recommended to limit or exclude foods rich in TMANO, such as fish, but it is not recommended to restrict choline, as this can cause liver and neurological damage.
In summary, dietary control is an important point in the treatment of trimethylaminuria. Additionally, dietary strategies and supplements such as riboflavin and probiotics can be used to lessen the impact of body odor. It is important that each patient knows how each food affects them and how odor changes with age, which will allow greater control of their odor and a better quality of life.
The exact cause of the bad odor cannot be determined without a medical diagnosis. However, maintaining good personal hygiene, using appropriate intimate soaps and consulting a specialist can help solve the problem.
